The film begins with Sam having a vision of the North Bay Bridge collapsing while he is on a bus with his co-workers for a corporate retreat. He manages to get off the bus with his girlfriend Molly Harper (Emma Bell), his best friend Peter Friedkin (Miles Fisher), Peter's girlfriend Candice Hooper (Ellen Wroe), his boss Dennis Lapman (David Koechner), and his colleagues Nathan Sears (Arlen Escarpeta), Isaac Palmer (P. J. Byrne), Olivia Castle (Jacqueline MacInnes Wood), and Sam's rival Roy Carson (Brent Stait). The bridge collapses as Sam predicted, killing everyone else on the bus and many other people.
At the memorial service, the group is approached by William Bludworth (Tony Todd), a mysterious coroner who warns them that death does not like to be cheated and will come for them in the order they were meant to die. He also hints that there may be a way to break the cycle by killing someone else and taking their life span.
Soon, the survivors start to die in bizarre and gruesome ways that resemble freak accidents. Candice is killed when her spine snaps during a gymnastics practice. Isaac is killed when his head is crushed by a falling Buddha statue at a spa. Olivia is killed when she falls out of a window after undergoing laser eye surgery. Roy is killed when he is impaled by a hook at a factory. Nathan accidentally kills Roy's replacement, John (Roman Podhora), and believes that he has saved himself.
Sam and Molly learn from Agent Jim Block (Courtney B. Vance), an FBI agent who is investigating the bridge collapse, that Nathan was supposed to die before Roy, meaning that he did not cheat death but only skipped his turn. They also realize that Peter was supposed to die before Candice, and that he has become unstable and obsessed with killing someone else to save himself.
Peter tries to kill Molly at a restaurant, but Sam intervenes and they fight. Dennis arrives at the scene and tries to stop them, but he is killed when a wrench flies into his head. Peter then shoots Sam in the stomach, but Molly shoots Peter in the head, killing him. Sam and Molly are taken to a hospital, where Sam undergoes surgery and survives.
Two weeks later, Sam and Molly board a plane to Paris for their honeymoon. They are shocked to discover that they are on Flight 180, the same plane that exploded in the first Final Destination movie. They try to get off the plane, but they are too late. The plane explodes, killing them and everyone else on board.
The film ends with a montage of the death scenes from all five movies, accompanied by AC/DC's "If You Want Blood (You've Got It)".
